HOSPICE CARE
TREATMENT
We Care for terminally ill with the aim of ensuring a focused and an improved quality of life during their end stage is very comfortable.
ELIGIBILITY
The loved one must be certified as terminally ill, and have decided not to pursue curative measures Loved one must elect hospice care with a specific hospice agency Patient and their family can decide to discuss hospice with the physician as the need arises.
CARE LOCATION
Generally home-based Free standing hospice centers, hospitals, nursing homes, and other long term care facilities.

PALLIATIVE CARE

TREATMENT
Alongside curative and life prolonging measures administered to those that have terminal and non-terminal illnesses Can occur at any stage of the disease progression
ELIGIBILITY
The Loved one must request palliative care referral from the administering physician
CARE LOCATION
Care provided to such clients are mostly carried out at institutions such as hospitals, nursing homes, assisted living facilities, personal care homes, respite care homes, or extended care facilities. “All hospice care is palliative but *not* all palliative is hospice care.
